Position Overview
We are seeking an experienced LVN/LPN to provide nursing care, medication administration, and clinical support to residents in a long-term care setting.
Required Qualifications
Active New York State LVN/LPN license, verified at submission.
Minimum 1 year of Long-Term Care (LTC)/Skilled Nursing experience.
Current BLS certification.
Experience caring for Alzheimer’s and dementia patients preferred/required for the patient population.
Successful completion of medication administration (med pass) test and clinical observation.
Strong medication administration and clinical assessment skills.
Strong attendance, punctuality, and reliability.
First-time travelers accepted.
Flexibility with scheduling needs.
Responsibilities
Administer medications and treatments safely according to physician orders.
Perform medication passes and monitor residents for effectiveness and adverse reactions.
Provide nursing care according to facility policies and established care plans.
Monitor resident conditions and report significant changes to appropriate healthcare providers.
Provide care to residents with Alzheimer’s, dementia, and other long-term care needs.
Complete accurate and timely clinical documentation.
Supervise CNAs and coordinate resident care activities.
Communicate effectively with residents, families, CNAs, nurses, providers, and interdisciplinary team members.
Maintain resident safety and follow infection-control and facility policies.
Support a positive, professional, and respectful environment for residents.
