Position Overview
We are seeking a Licensed Practical Nurse to provide nursing care to residents in a long-term care and skilled nursing setting, with potential assignments in the Transitional Care Unit (TCU) based on clinical competency.
Responsibilities
Provide nursing care to residents in a long-term care and skilled nursing setting.
Monitor residents' health conditions and report changes to the healthcare team.
Administer medications and treatments according to physician orders and facility protocols.
Perform nursing assessments and assist with individualized care plans.
Provide wound care and IV therapy when qualified and assigned.
Maintain accurate and timely clinical documentation.
Collaborate with registered nurses, physicians, and other healthcare professionals.
Float between LTC and TCU units according to clinical competency and facility needs.
Promote resident safety and follow infection control standards.
Required Qualifications
Active LPN license.
Ability to work 12-hour shifts and variable schedules, including weekends.
Ability to float between facility units based on clinical competency.
Strong clinical assessment, communication, and documentation skills.
Medication administration and resident care experience.
Preferred Qualifications
Long-Term Care (LTC) or Skilled Nursing Facility experience.
Transitional Care Unit (TCU) experience.
Wound vac management experience.
IV therapy experience.
Step-down or higher-acuity patient care experience.
