Position Overview
We are seeking a Licensed Practical Nurse (LPN) to provide nursing care in long-term care and rehabilitation settings. The ideal candidate will have experience in long-term care or skilled nursing and the flexibility to work variable evening and night shifts.
Required Qualifications
Active LPN license.
Experience in long-term care or skilled nursing.
Availability for variable evening/night shifts.
Ability to meet facility onboarding and compliance requirements.
Responsibilities
Provide nursing care to residents in long-term care and rehabilitation settings.
Monitor residents' conditions and report changes to the appropriate healthcare team.
Administer medications and treatments according to provider orders and facility policies.
Maintain accurate resident documentation using the facility's current paper-charting process.
Support residents in rehabilitation, orthopedic recovery, long-term care, and dementia care.
Coordinate with RNs, CNAs, and other care team members.
Follow resident safety, infection prevention, and facility compliance policies.
Additional Information
The facility has rehabilitation, long-term care, and dementia-care units.
The dementia unit is not locked; the facility has alarmed doors.
Paper charting is currently used, with a planned transition to an electronic medication administration system.
