Position Overview
We are seeking a Registered Nurse (RN) / Case Manager to coordinate acute care case management, discharge planning, and transitions of care. The ideal candidate will have California case management experience, strong care coordination skills, and the ability to collaborate with interdisciplinary healthcare teams.
Required Qualifications
Active California RN license required at submission. Pending licenses are not accepted.
Minimum 3 years of acute care case management experience.
California case management experience required.
Current Basic Life Support (BLS) certification.
Flu vaccination required; exemptions are not accepted.
EPIC experience highly preferred.
Ability to work Monday–Friday, 8:00 AM–5:00 PM.
No scheduling requests can be accommodated.
Responsibilities
Provide case management services for acute care patients.
Coordinate discharge planning and transitions of care.
Assess patient needs and develop appropriate discharge plans.
Collaborate with physicians, nurses, social workers, patients, families, and other healthcare professionals.
Facilitate appropriate utilization of healthcare resources.
Coordinate post-acute services and follow-up care.
Monitor patient progress and identify barriers to discharge.
Maintain accurate and timely case management documentation.
Support effective communication and continuity of care throughout the patient's hospitalization.
Follow hospital policies, regulatory requirements, and case management standards.
Perform other related duties as assigned.
