Position Overview
We are seeking a dedicated Physical Therapist to provide rehabilitation services in a skilled nursing facility, helping patients improve mobility, functional independence, and overall physical function.
Required Qualifications
Graduate of an accredited Physical Therapy program.
Current New Jersey Physical Therapist license or eligibility for licensure.
Strong clinical assessment and treatment skills.
Preferred Qualifications
Experience in a skilled nursing facility (SNF), rehabilitation, or post-acute care setting.
Familiarity with geriatric and medically complex patient populations.
Responsibilities
Evaluate patients and develop individualized treatment plans.
Implement evidence-based physical therapy interventions to improve mobility, strength, balance, and functional independence.
Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and interdisciplinary team members.
Monitor patient progress and modify treatment plans as appropriate.
Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines.
Educate patients, families, and caregivers on treatment plans and home exercise programs.
Ensure compliance with regulatory requirements and safety standards.
