Total Weekly Pay: $2,449.00
We are seeking an experienced Master of Social Work (MSW) professional to support the Case Management department in an acute-care hospital setting. The ideal candidate will have acute-care experience and the ability to coordinate patient care, support discharge planning, and collaborate with multidisciplinary healthcare teams.
Support case management activities in an acute-care hospital environment.
Assess patient needs and assist with care coordination and discharge planning.
Collaborate with physicians, nurses, social workers, patients, families, and other healthcare professionals.
Help facilitate safe patient transitions and continuity of care.
Manage an approximate caseload of 20–23 patients.
Maintain accurate documentation and follow established hospital policies and procedures.
Float to the Laguna facility as required.
Master’s degree in Social Work (MSW).
Minimum of 1 full year of acute-care experience.
Previous travel assignment experience required; first-time travelers will not be accepted.
California state license is not required.
