Position Overview
We are seeking a Licensed Master Social Worker (LMSW) to provide social work services in an inpatient hospital setting, including psychosocial assessments, discharge planning, and support for patients and families.
Required Qualifications
Master’s Degree in Social Work (MSW).
Active New York State LMSW license.
Inpatient hospital social work experience.
Discharge planning experience.
Experience with goals-of-care and end-of-life planning conversations.
Strong communication skills and experience conducting psychosocial assessments.
Oncology experience preferred.
Responsibilities
Provide social work services to patients and families in an inpatient hospital setting.
Conduct psychosocial assessments and identify patient and family needs.
Assist with discharge planning and coordinate appropriate resources.
Participate in goals-of-care discussions and end-of-life planning.
Support patients and families through complex medical situations.
Collaborate with physicians, nurses, case managers, and other members of the care team.
Provide referrals and connect patients with appropriate community resources.
Document assessments, interventions, and care planning activities.
Support oncology patients and their families as appropriate.
