Travel: $2,340–$2,440/week
Local: $2,000–$2,100/week
Job Description:
Provide medical social work services in an acute care hospital, including psychosocial assessments, discharge planning, care coordination, patient support, and connecting patients with post-acute and community resources.
Responsibilities:
Perform psychosocial assessments.
Develop and coordinate discharge plans.
Assist with post-acute care planning.
Connect patients with community resources.
Identify and address barriers to safe discharge.
Coordinate with physicians, nurses, case managers, and other healthcare staff.
Support patients and families.
Maintain accurate clinical documentation.
Participate in interdisciplinary care planning.
Required Qualifications:
Master of Social Work (MSW) required.
Minimum 2 years of acute medical Social Work experience.
Social Work and Discharge Planning experience required.
Acute Care hospital experience required.
Strong patient assessment and care coordination skills.
First-time travelers accepted with additional screening.
