Pay Rate:
Traveler: $45–$50/hr
Required Qualifications:
Active Washington MSW license — Required
Minimum 1 year of acute-care social work experience — Required
Previous travel experience — Required
Discharge planning and care coordination experience — Required
BLS certification — Required
Epic experience — Required
AIDA EMR experience — Preferred
Strong case-management experience in Med-Surg, LLOS, and ED — Preferred
Strong psychosocial assessment, crisis intervention, and communication skills
Ability to work effectively with patients, families, and interdisciplinary teams
Responsibilities:
Conduct comprehensive psychosocial assessments for hospitalized patients
Develop and coordinate discharge plans and transitions of care
Collaborate with physicians, nurses, case managers, and community agencies
Provide crisis intervention, counseling, and patient/family education
Arrange referrals for rehabilitation, home health, hospice, and community resources
Support patients and families with social, emotional, and discharge-related needs
Maintain accurate and timely documentation in Epic
Coordinate care with community and healthcare resources
Participate in interdisciplinary care planning
Float between hospital campuses and inpatient units as required
Follow hospital policies, patient-safety standards, and social-work practices
